  • photographic proof of identity – passport, driving licence (not mandatory)
  • proof of address – utility bill, bank statement (not mandatory)
  • your NHS number (this is not mandatory, but will make things easier).

Temporary Patient Registrations

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to contact the surgery and complete and return this form

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours, but less than three months.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Non-urgent advice: Please note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number

Important Information for New Patients

If you are registering with our practice and currently receive repeat medications or are under a Shared Care Agreement (SCA) with a specialist, please note the following:

Shared Care Agreements

  • If you are receiving specialist medication under a Shared Care Agreement, we require formal documentation from your hospital consultant or specialist.
  • This must include:
    • Details of the medication and dosage
    • Monitoring requirements and frequency
    • Documentation from your last review 
  • Please be aware that Shared Care is a voluntary arrangement. Our clinicians must assess whether it is safe and appropriate to take over prescribing and monitoring responsibilities.

Repeat Prescriptions

  • For any repeat medications, we require:
    • recent prescription history from your previous GP or specialist
    • Relevant clinical notes or hospital discharge summaries where applicable
  • Without appropriate documentation, we may not be able to continue prescribing until a clinical review has taken place.

Please Note:

  • We cannot accept Shared Care requests from private providers unless agreed and supported by NHS commissioning arrangements.
  • Prescribing decisions are made based on clinical safety, capacity, and national guidance.
