If you have moved but are still living within the practice area please let us know:
- tell us at reception
- call The Surgery (0117 965 3102)
- complete the Notification of Change of Address Form and bring it into The Surgery or send to [email protected]
- change your address online here
If you are not sure whether you still live in the practice area please ask reception or see our Surgery boundary map here
If you are moving outside the practice area but would like to stay registered here you will need to fill in a form for registering as an ‘Out of Area Patient’. The decision as to whether or not you are able to be registered will depend on suitability of location and any healthcare needs you may have. This is due to doctors being unable to provide home visits for Out of Area patients. It will take around ten days to process these requests and you will be informed by letter.
