If you have moved but you are still a student at UWE, you can remain registered with us, even if you no longer live in the practice boundaries.

If you have changed your address please let us know, you can:

  • complete the online change of address form available below
  • use our online tool Klinik which is here (option General Enquiries/Update your details)
  • download paperwork form, MS Word file here or the PDF version here, fill it in and provide it to us: by person, by post, send it to [email protected] or upload it here
  • visit UWE Health Centre and speak with the reception (or our main site The Surgery in Fishponds, The Surgery location is here)
  • call UWE Health Centre on 0117 328 6666
