I have cancelled my holiday

If you need to cancel your holiday due to illness, contact your insurance company, who will provide you with the appropriate form (paper or electronic version).

Please provide the completed form to The Surgery by the person (to a receptionist), by post, by email here or upload online here, and this will be forwarded to a GP to complete. Please provide a clear explanation of your circumstances.

You need to allow five working days for this to be completed and there is a charge for this service as this is non-NHS work. Fees are available here.

The form will be available once payment is received.
